What to expect

What to bring

• Completed application form Proof of address Proof of income received by any member of the household during the previous 30 days. Examples of acceptable verification are pay stubs

Proof of ALL liquid assets (verbal or written) Proof of the applicant's change in circumstance leading to the current need and written proof that the applicant made a significant attempt to use other community services to meet the need presented. If pregnant
• need a medical statement verifying pregnancy. In most cases
• will need a disconnect notice (see WHO QUALIFIES for exceptions). For water bill: if the applicant's name is not on the bill
• will need a copy of lease stating that tenant is responsible to pay water bill. In addition
• if the cost of the documented need exceeds the amount of PRC
