- You can get help once every 30 days if you live in Zip Code 44105 and your income is 200% of the Federal Poverty Limit or less. If your income is higher but you have a financial hardship, you might still get help.

- Proof of Address/Residency (this could be a current postmarked utility bill

- or other official document - such as that received from Social Security). Photo I.D. if available. However
