Who can use it
You qualify if your income is at or below 200% of the Federal Poverty Guidelines. If you are 60 or older, you can apply. If you are under 60, you must be homebound due to a physical condition and have a referral from a Social Worker. In Cleveland, the service only covers Old Brooklyn (zip codes 44109 and 44144). The service also covers Parma, Parma Heights, Brook Park, Middleburg Heights, Brooklyn, and parts of North Royalton and Berea.
Languages
English on-site. Other languages by phone.
What to expect
Call Tuesday through Saturday, 10:00am to 2:00pm, to set up a delivery time. If no one answers, leave a message and they will call you back within 1-3 business days. If you are under 60, you need a Social Worker to write a letter saying you have a physical disability that keeps you homebound.
What to bring
Call ahead to confirm what documents you'll need to bring.
