Who can use it

You must be 60 or older, or 18-59 with a disability, and live in a private home in Cuyahoga County. You can't be eligible for Medicaid waiver programs.

They can coordinate things like medical transportation, personal care, home-delivered meals, and even small bathroom modifications for safety.
