You must live in Ashtabula, Cuyahoga, Geauga, Lake, Lorain, or Medina Counties and have an AIDS/HIV diagnosis.
Languages
English, Spanish on-site. Other languages by phone.

  • Birth Certificate
  • ID's for each family member
  • proof of income
