Who can use it

You must live in Ashtabula, Cuyahoga, Geauga, Lake, Lorain, or Medina Counties and have an AIDS/HIV diagnosis.

Birth Certificate
ID's for each family member
proof of income
etc. Rental assistance may require an eviction notice. Utility assistance requires a shut-off notice.
