This service gives one-on-one help to older adults and people with disabilities. They'll help you find community resources, benefit programs, and other services.

M-F: 8:00am-5:00pm through call center.
Who can use it: You must live in Cuyahoga, Geauga, Medina, Lake, or Lorain County. You need to be 60 or older, or 18-59 with a disability. They can also help caregivers and family members.
Languages: English on-site. Other languages by phone.
What to expect: You'll work with someone who will help you understand what's available. They'll make sure you know your options for living, care, and support.
