NUTRITION INFORMATION / SUPPLEMENTAL FOOD PROGRAM

They give out food vouchers and teach you about nutrition. They also offer support and referrals to eligible women, children, fathers, and caregivers. Nursing moms can get breastfeeding help.

By appointment: Chardon Office: Second & Fourth Friday of the month: 8:00am-4:30pm; Second Tuesday of the month: 8:00am-4:30pm; Fourth Wednesday of the month: 9:30am-6:00pm. Middlefield Office: First and third Friday of the month: 8:00am-4:30pm.
Who can use it
This is for pregnant women, or women up to six months after birth, miscarriage, or abortion. Both mom and baby get help until the baby is one, then the child gets help until age five. Caregivers of children under five can also get help.

What to expect
You need to make an appointment. The Chardon office is open on the second and fourth Friday of the month from 8:00 AM to 4:30 PM, the second Tuesday of the month from 8:00 AM to 4:30 PM, and the fourth Wednesday of the month from 9:30 AM to 6:00 PM.
What to bring
  • ID that shows name and birth date for those in the household who are applying for benefits (for the baby
  • will accept a crib card from the hospital). May need proof of pregnancy from doctor or clinic
  • unless the pregnancy is clearly visible. For income verification
  • will accept the following: three pay stubs
  • SSI statements
  • Medicaid card
  • proof of enrollment in Ohio Works First
  • Food Assistance Card
  • a written statement from child's father saying he is paying child support. Will also need proof of the child's current immunizations and proof of residency (current utility
  • telephone
  • or credit card statement). Those who are not the mother
  • but who have custody of the child (such as fathers and kinship caregivers) will need proof of custody (this may vary - agency will advise).
