This is for income-eligible pregnant and breastfeeding women, women who recently had a baby, infants, and children up to 5 years old. You need to be an Ohio resident.

  • ID that shows name and birth date for the person applying for WIC
  • Proof of residency (a recent utility bill or piece of mail with address is acceptable)
  • Proof of pregnancy (if applicable)
  • Proof of income: acceptable proofs include 3 most recent pay stubs.
