NUTRITION INFORMATION / SUPPLEMENTAL FOOD PROGRAM

They give you food vouchers and teach you about good nutrition. They also offer support for breastfeeding and referrals to other services.

By appointment: Chardon Office: Second & Fourth Friday of the month: 8:00am-4:30pm; Second Tuesday of the month: 8:00am-4:30pm; Fourth Wednesday of the month: 9:30am-6:00pm. Middlefield Office: First and third Friday of the month: 8:00am-4:30pm.
Who can use it
You must be pregnant, or a new mother (up to six months postpartum). Children can receive assistance until their fifth birthday. Fathers and kinship caregivers can also use this service.

What to expect
During the summer, you might get extra vouchers for fresh fruits and vegetables at local farmers' markets. They also offer child car safety seats at a reduced price.
What to bring
  • ID that shows name and birth date for those in the household who are applying for benefits (for the baby
  • will accept a crib card from the hospital). May need proof of pregnancy from doctor or clinic
  • unless the pregnancy is clearly visible. For income verification
  • will accept the following: three pay stubs
  • SSI statements
  • Medicaid card
  • proof of enrollment in Ohio Works First
  • Food Assistance Card
  • a written statement from child's father saying he is paying child support. Will also need proof of the child's current immunizations and proof of residency (current utility
  • telephone
  • or credit card statement). Those who are not the mother
  • but who have custody of the child (such as fathers and kinship caregivers) will need proof of custody (this may vary - agency will advise).
