You must be an ex-offender in Cuyahoga County, referred directly by prisons or courts, with a documented disability, and experiencing homelessness upon release.

You'll be referred directly through prisons or courts. You'll need to show proof of a documented disability.
What to bring
  • Completed application and supporting documents
  • which include verification of imprisonment
  • verification of all household income
  • copies of valid photo ID's
  • birth certificates and social security cards for all household members.
