Who can use it

You must be an ex-offender in Cuyahoga County, referred directly by prisons or courts, with a documented disability, and experiencing homelessness upon release.

You'll be referred directly through prisons or courts. You'll need to show proof of a documented disability.

What to bring

Completed application and supporting documents
which include verification of imprisonment
verification of all household income
copies of valid photo ID's
birth certificates and social security cards for all household members.
