What to bring

Photo I.D. for all adults over the age of 18 in the household
social security cards or birth certificates for all household members
income documentation for all adults over the age of 18 in the household. Additional documentation may include mortgage statement
foreclosure documentation
and homeowner's Insurance declaration page
if applicable.
