NUTRITION INFORMATION / SUPPLEMENTAL FOOD PROGRAM

This program gives you a food card for groceries, offers nutrition and breastfeeding help, and connects you to other services. In the summer, some people can also get a voucher for fresh produce at farmers markets.

Who can use it
You qualify if you are pregnant, or up to six months postpartum. Your child can receive help until their fifth birthday. Fathers and other legal guardians with custody of a child also qualify.

What to bring
  • I.D. that shows name and birth date for those in the household who are applying for benefits. (Could even be a crib card from the hospital). Need proof of pregnancy from doctor or clinic
  • unless the pregnancy is clearly visible. Also need 3 pay stubs
  • SSI statements
  • proof of Medicaid
  • Ohio Works First
  • Food Stamps or other means of verifying income
  • a written statement from child's father saying he is paying child support
  • proof of child's current immunizations
  • proof of residency (including utility
  • telephone or credit card statement).
