Who can use it
You must be a pregnant woman, or have given birth, miscarried, or had an abortion in the last six months. Children are covered until their fifth birthday.

What to expect
You’ll also get nutrition education, breastfeeding support, and referral services. In the summer, some people get a $30 voucher for farmers' markets while supplies last.
What to bring
  • I.D. that shows name and birth date for those in the household who are applying for benefits. (Could even be a crib card from the hospital). Need proof of pregnancy from doctor or clinic
  • unless the pregnancy is clearly visible. Also need 3 pay stubs
  • SSI statements
  • proof of Medicaid
  • Ohio Works First
  • Food Stamps or other means of verifying income
  • a written statement from child's father saying he is paying child support
  • proof of child's current immunizations
  • proof of residency (including utility
  • telephone or credit card statement).
